How the Prefix System Works

Every BCBS insurance card starts with a 3-letter alpha prefix. This prefix identifies which of the 34 independent BCBS companies issued the plan. Providers use the prefix to route claims to the correct company. Without it, a claim submitted to BCBS of Texas would fail if the patient actually has BCBS of Michigan.

Unlike other prefix lists that only show the prefix and plan name, each entry in our database includes the provider services phone number, plan type (PPO, HMO, or TRAD), and pre-certification contact. This data comes from official provider reference documents.

What Do the 3 Letters in a BCBS Prefix Mean?

The 3 characters are an alphanumeric code (letters and numbers) assigned by individual BCBS companies to identify specific plans. The first character often indicates the plan category or company. The second and third characters further narrow the product. Not every possible combination is used. Our database contains over 21,000 unique prefix-to-plan assignments because many prefixes are shared across multiple employer groups or account types within the same BCBS company.

Are BCBS Prefixes the Same Across All States?

No. Each of the 34 BCBS companies assigns its own unique prefixes. Prefix ZGP belongs to BCBS of Illinois, not BCBS of Texas. Two different states will never share the same prefix for different companies. This is how the BlueCard system routes claims correctly when you visit a provider in another state. How Often Are New BCBS Prefixes Added?

BCBS companies add new prefixes whenever they create new plan products or sign new employer group contracts. There’s no fixed schedule. Our database is updated regularly. If you have a brand-new card and your prefix isn’t listed, call the member services number on the back of your card.

BCBS companies also periodically retire or replace prefixes. This usually happens when a company migrates to a new data platform or restructures plan offerings. If the prefix on your physical card doesn’t match what your online portal shows, the portal is correct. The physical card is outdated.
